Dental Coverage
80% Basic no deductible
  $25 single / $50 family deductible
  $700 maximum per person per year

Covers repairs, rebasing, relining of existing dentures. 6 month recall.

Eye exams up to $50 per adult every 24 months and $50/child every 48 months.

Vision Care $200 every 24 months (12 months for children)


Weekly Indemnity (Short Term Disability)
Options Benefits from the 15th day of disability for up to 15 weeks
  Benefits from the 31st day of disability for up to 13 weeks
  Benefits from the 1st day of accident, 8th day of illness, for up to 17 weeks
Replaces 66 2/3 of weekly earnings to a maximum benefit of $800 per week during brief involuntary absences from work.
Benefits resume immediately if disability recurs within 14 days of return to work.


Long Term Disability
Options Benefits from the 121st day of disability for up to 2 years
  Benefits from the 121st day of disability for up to 5 years
  Benefits from the 121st day of accident, to age 65
Plans with 100% employee-paid premiums pay non-taxable benefits.
Benefits includes Posaction Plus, a confidential counseling service, and Coach-Tel, a telephone service that helps owners and managers address human resource issues.


Dependents Insurance
Options Spousal coverage of $5,000 and $2,500 per child
  Spousal coverage of $10,000 and $5,000 per child
No evidence of insurability required.


Critical Illness
Options $25,000
Coverage reduces at age 55 to $12,500 and terminates at age 65.


Life Insurance / Accidental Death & Dismemberment (Mandatory)
Options Level $25,000 benefit
  1 x Annual Salary
Coverage reduces at age 65 to 25% of the face value and terminates at age 75.
